Dementia Coordinator - Full Time


Kings Harbor Multicare Center is a 720 bed LTC/Sub-Acute/Rehabilitation facility located conveniently in the N.E. Bronx off 2 major highways with ample parking.? We are a 3 building campus with one building dedicated to Alzheimer/Dementia residents.? We strive to offer the best care and to create meaningful relationships with our residents and their families.


Responsibilities include:


Possess the ability to design programs for all different levels of dementia


Engage and motivate residents to participate in different programs


Be a creative thinker with strong leadership skills that is able to guide and teach various staff levels in regards to different types of Dementia and the impact that participation in various programs has on residents' lives


Must be organized, able to create, initiate and implement policies and procedures regarding dementia care in the long term care environment


Be a kind, compassionate individual who cares about their work and is able to work with our interdisciplinary team


Should be up to date on new trends in care and knowledgeable on the CM regulations regarding dementia care and best practices




Qualifications?


BS or BA in Therapeutic Recreation or an allied field


CDP certification a must


CADDCT preferred


Bi-lingual in Spanish preferred
